Are GLP-1 Pills as Effective as the Injections?

The answer is: they can be highly effective, but it depends on the specific medication, dosage, and individual patient. Pills and injections should not necessarily be viewed as interchangeable versions of the same treatment.

How Do GLP-1 Medications Work?

GLP-1 receptor agonists work by mimicking the effects of a naturally occurring hormone involved in appetite regulation, blood sugar control, and digestion. Injectable GLP-1 medications have been extensively studied for weight management. For example, a large clinical trial of once-weekly injectable semaglutide found an average weight reduction of approximately 15% after 68 weeks when combined with lifestyle intervention.

Oral medications work through the same general biological pathway in the case of GLP-1 receptor agonists, but the medication’s formulation, dosage, absorption, and dosing schedule can affect its results.

How Effective Are GLP-1 Pills?

Recent research shows that oral GLP-1 medications can produce substantial weight loss. In 2022, Novo Nordisk coordinated the OASIS 4 (Oral Semaglutide Treatment Effect in People with Obesity) clinical trial. Its primary purpose was to evaluate the efficacy and safety of a once-daily oral semaglutide tablet (25 mg dose) for chronic weight management. Adults with overweight or obesity who took this dose experienced an estimated average weight reduction of 13.6% after 64 weeks, compared with 2.2% with placebo. Gastrointestinal side effects were common, as they are with GLP-1 medications generally.

Another oral GLP-1 medication, orforglipron, has also demonstrated clinically meaningful weight reduction in Phase 3 trials. The medication was approved by the FDA in 2026 as Foundayo™ for chronic weight management in adults with obesity or adults with overweight who have at least one weight-related condition.

These results demonstrate that oral GLP-1 therapy can be a legitimate medical option for weight management — not simply a less-effective alternative to injections.

Does That Mean GLP-1 Pills Work Just as Well as Injections?

Not necessarily. One important point is that researchers cannot simply compare the percentage of weight loss from two separate clinical trials and conclude that one medication is equivalent to another. The participants, doses, study designs, and other factors can differ.

For example, the OASIS 4 oral semaglutide study did not directly compare the 25-mg oral dose with injectable semaglutide. A published commentary noted that the absence of an active-comparator group limits the ability to determine how the oral formulation compares directly with injectable semaglutide.

There are also differences among individual medications. Some injectable treatments have produced average weight losses greater than those observed with some oral options, while newer oral medications are expanding the possibilities for people who would rather not use injections.

In other words, “pill versus injection” is not the only question that matters. The specific medication and the individual taking it matter, as well.

Why Might Someone Prefer a GLP-1 Pill?

For some patients, the biggest advantage of an oral medication is simple: no injections. Someone who dislikes needles or does not want to administer a weekly injection may find a daily tablet easier to incorporate into their routine. Oral treatment may also make medical weight management more appealing to people who have been hesitant about injectable medications.

However, oral medications aren’t automatically easier for everyone. Depending on the specific medication, there may be instructions concerning when and how the pill should be taken. Other oral GLP-1 medications have different dosing requirements. That’s why following the prescribed instructions is important.

What About Side Effects?

Both oral and injectable GLP-1 medications can cause gastrointestinal side effects. Depending on the medication, these may include nausea, vomiting, diarrhea, constipation, abdominal discomfort, or other digestive symptoms. In the OASIS 4 trial, gastrointestinal adverse events occurred in 74% of participants receiving oral semaglutide compared with 42.2% receiving placebo.

Which Is Better: A GLP-1 Pill or an Injection?

There isn’t one answer that applies to everyone. For some people, an injectable medication may provide the degree of weight loss they are looking for. For others, an oral medication may be an attractive option because they prefer taking a pill rather than giving themselves an injection.

Other considerations can include:

  • Your weight-loss goals
  • Other medications you take

“The pills do work, but overall, I still think the injections are more effective,” says William E.
